American Academy of Audiology
Idaho State University Audiology Program is approved by the American Academy of Audiology to offer Academy CEUs for this activity. The program is worth a maximum of .9 CEUs. Academy approval of this continuing education activity is based on course content only and does not imply endorsement of course content, specific products, or clinical procedure, or adherence of the event to the Academy’s Code of Ethics. American Speech-Language-Hearing Association
Idaho Speech, Language, Hearing Association is approved by the Continuing Education Board of the American Speech-Language-Hearing Association (ASHA) to provide continuing education activities in speech-language pathology and audiology. The total continuing education units (CEUs) registered for this event is .95 CEUs or 9.75 hours. The general convention sessions are registered for 0.95 ASHA CEUs at various instructional levels and content areas. ASHA CE Provider approval does not imply endorsement of course content, specific products, or clinical procedures.

Statement of Satisfactory Completion:
Disclosure Statements will be read by each presenter at the beginning of each session during course Introductions. Through our registration process, which is mandatory, ISHA will collect each learner’s information, including: Name, State license number, Discipline, ASHA number, Email, Phone, and Home address. ISHA will send an email advertising the convention, including the registration link, to all ISHA members.
To document course completion, each learner will sign in at the beginning of each session as well as sign out upon each session completion. At the end of the convention, or when the learner completes on demand viewing, each participant must complete the participant evaluation form and learner evaluation for each session attended. The session is not considered attended without this information. ISHA will maintain all information related to the course planning and registration for 4 years. Complaints can be submitted in writing via email to: idahospeechhearing@gmail.com Complaints must include the complainant's name, contact information, and a detailed description of the issue including the date of the event. Cancellation requests must be submitted in writing at least 15 days prior to the event or service date for a full refund. If a cancellation is received less than 15 days before the event or service, a 50% refund will be issued. No refunds will be provided for cancellations made on the day of the event or after the event has commenced.
